Tuberculosis resistant to first-line drugs

Renal and hepatic impairment

Renal impairment

Creatinine Clearance (mL/min) Oral Dose
50 and over Dose as in normal renal function
10-50 250mg-500mg every 24 hours. Monitor blood levels weekly.
Less than 10 250mg-500mg every 36-48 hours. Monitor blood levels weekly.
HD/HDF/High Flux

 Dialysed; give after dialysis

Dose as in creatinine clearance less than 10mL/min or 500mg three times a week after dialysis.

PD

Unknown dialysability

Dose as in creatinine clearance less than 10mL/min. 

Note: dose ranges use creatinine clearance, rather than eGFR.

 

Hepatic impairment

No dose adjustment required but monitor hepatic function during treatment.

Additional information

Cycloserine levels should be monitored in all patients, especially in those with renal impairment or if dose exceeds 500 mg daily or if signs of toxicity. See Cycloserine - TB DRUG MONOGRAPHS for more information.
