Standard Dose

  Intravenous dose
Standard dose

Loading dose: 100mg single dose

followed 12 hours later by

Maintenance dose: 50mg BD

Obesity

No dose adjustment usually required.

In morbidly obese patients with multi-drug resistant infections: Higher doses may be required, discuss with Micro/ID.

Renal and hepatic impairment

Renal impairment

  • No dose adjustment required.
  • HDF/High Flux - Dose as in normal renal function. Unknown dialysability.
  • PD - Dose as in normal renal function. Not dialysed.

 

Hepatic impairment

  • Mild and moderate hepatic impairment: No dose adjustment required.
  • Severe hepatic impairment (Child- Pugh Class C): Use with caution. Give 100mg loading dose followed by 25mg BD.
